Integrated Community Supports (ICS)

Overview

Integrated Community Supports (ICS) is a service available under Minnesota’s Home and Community-Based Services (HCBS) waivers. ICS provides housing support and individualized assistance for people with disabilities or seniors who live in their own home or apartment and need help with day-to-day living.

The goal is to promote independence, safety, and community inclusion while ensuring individuals can thrive in a stable living environment.

What ICS Includes

Our team helps with a wide range of supports, such as:

assistance with cooking, cleaning, shopping, and organizing daily tasks

Health, safety, and wellness

support in managing medications, accessing healthcare, and following health routines

Community living skills

help building social connections, accessing community resources, and navigating transportation

Problem-solving and self-advocacy

coaching to strengthen decision-making skills and promote independence

Support planning

guidance to achieve personal goals in education, employment, and daily living

Who Is Eligible?

ICS is available to people who:

Receive Medical Assistance (MA) and an HCBS waiver

Z

Live in an eligible ICS setting (such as an apartment or other DHS-approved housing)

Z

Need daily support to live as independently as possible in the community
