At Grandview Reliable Care, we are licensed by the Minnesota Department of Human Services to provide 245D Basic Services that help individuals live safely, independently, and with dignity in their homes and communities. These services focus on daily living support, supervision, and personal assistance without intensive medical or behavioral care.
Types of Basic Services
24-Hours Emergency Assistance
Provides immediate access to trained staff 24/7, either by phone or in person, during urgent situations. Whether it’s a medical concern, safety risk, or unexpected crisis, help is always available to ensure safety and peace of mind.
Adult Companion Services
Delivers non-medical care, supervision, and companionship to promote independence and reduce isolation. Companions may accompany individuals to community activities, support hobbies, and assist with light daily tasks.
Homemaker Services (Basic)
Supports a clean and comfortable home environment by helping with light housekeeping, laundry, meal preparation, and other household tasks. This service allows individuals to live safely and independently in their own homes.
Individual Community Living Support (ICLS)
Provides guidance and assistance with daily routines, skill development, health and safety monitoring, and community engagement. ICLS helps individuals maintain independence while building confidence in everyday living.
Night Supervision Services
Ensures awake staff are available throughout the night to respond to emergencies, assist with mobility, and provide safety support for individuals who cannot be left alone overnight.
Respite Care Services
Offers short-term care in the individual’s home or another approved location, giving regular caregivers time to rest and recharge while ensuring their loved one continues to receive quality support.
